1.     Press the “Web Browser” icon to open it.
2.     Type the URL “forms.office.com” on your web browser and tap Enter key.
3.     Then, sign in with any of the following subscription plans you have:
·        Office 365 school account
·        Office 365 work account
·        Microsoft account.
4.     Under “My Forms” section, click on the “New Form” to creating your form.
5.     Type a name of your Form that you want to give. If you want then give an optional subtitle.
6.     Choose the “Questions” section.
7.     Press the “Add Questions” tab to add a new question to that form which is created by you.
8.     Now, select from the Choice, Text, Rating, or any Date questions.
9.     Click on the “More” menu options for choosing additional options like Ranking, Likert, Net Promoter Score Questions.

10.                        If you have chosen “Choice” questions, type the content which you want to reveal for the questions and also all choices.
11.                        Again, click on the “Add Question” option to add some additional questions to your form.
12.                        If the questions are not in the right sequence and you want to modify it, modify the sequence by using the Up or Down arrows located right side of every questions.
13.                        Lastly, Click on the “Preview” appears at the top of the screen to easily take a snap of the form available on your computer.
14.                        Finally, before submitting a form, once test your form; provide the answers of every question in the preview mode. If it works, click on the “Submit” option to submit the form.
